opportunity to join our team as a LPN - NYU Langone Dialysis Center
- Mineola - (Outpatient Dialysis), Days. In this role, the
successful candidate Responsible for using his/her clinical
experience to plan the delivery of safe, therapeutic, age
appropriate patient care through planning, implementation and
evaluation over a scope of practice. With the patient and family
central to the care, the LPN incorporates the unique
characteristics and needs of each patient and family member into
the care delivery process. Works collaboratively within a
multidisciplinary health team to promote wellness and optimizes
patient outcomes. Performs multiple clinical and clerical duties as
required. Complies with established policies, procedures, rules and
regulations as they pertain to the hospital as well as federal
state and local regulations. Diligently observe all HIPPA
guidelines. Performs all other duties as it relates to specialty.
Job Responsibilities: Perform other duties as assigned. Completes
documentation reflecting multidisciplinary coordination. Documents
teaching/learning needs of patients and families while on program
and in preparation for discharge. Evaluates and documents patient's
response to nursing and multidisciplinary team interventions,
including patient/family teaching. Evaluates patient's clinical
status and attempts satisfactory resolution of patient clinical
issues at point of care prior to escalating issues. Takes action to
optimize achievement of patient outcomes. Participates in
identification, investigation and resolution of actual and/or
potential risk management /quality care/ethical issues. Addresses
patient issues following established communication hierarchy.
Evaluates patient progress toward health goals and modifies
critical path or plan or care accordingly. Interacts and
collaborates with other members of health team to identify specific
patient needs. Initiates appropriate interventions to amend
identified variances from plan of care. Documents patient and
family response to interventions and collaborate with the RN to
modify the plan of care as needed. Provides direct patient care
which includes but not limited to utilizing WUH policy/procedures,
patient care guidelines, physician s orders etc. Incorporates
standards of practice, clinical pathways, age-specific guidelines
and patient care guidelines into plan of care. Recognizes and
responds to issues that may cause problems and/or refers issues to
the RN; utilizes the escalation policy. Recognizes the family as
defined by the patient and incorporates the principles of family
centered care in daily practice. Identifies patient
satisfaction/dissatisfaction with plan of care; escalates issues as
required. Consistently verifies and documents patient advance care
directives for assigned patients. Maintains open channels of
communication through intra and inter shift reporting. Utilizes
appropriate communication in all interactions; demonstrates proper
telephone etiquette. Actively participates in maintaining a clean,
safe, therapeutic environment, including security of patient
belongings. Responds to emergency situations as appropriate.
Ensures appropriate emergency equipment is present and operational.
Is cognizant of HEICS plan and participates in implementing plan as
required. Obtains prior authorization for medication from patient s
insurance company as needed Obtains referrals for patient tests
ordered by MD as per patient's insurance requirements. Maintains
exam room for necessary supplies, cleanliness. Processes medication
orders and refills through the Workflow system. Verifies dosage and
medication using various tools progress notes, Clarity, physician
or contacting patient if necessary. Enters comments and your
initials in Workflow when receiving verbal approval from a
physician to refill a prescription. Identifies patient
satisfaction/dissatisfaction with plan of care. If patient
expresses dissatisfaction, implements conflict resolution skills
and addresses issues with plan of care. Assess and documents
patient/family advance care directives and Rights and
Responsibilities if indicated. Establishes collaborative and
collegial relationships with health care team members. Minimum
Qualifications: To qualify you must have a High School or
Equivalent Technical Diploma Graduate of accredited school of
licensed practical nursing 0 - 1 year nursing experience working in
a hospital, ambulatory, private practice or home health care
setting preferred. Required Licenses: LPN Qualified candidates must
be able to effectively communicate with all levels of the

- $84,207.84 Annually. Actual salaries depend on a variety of
factors, including experience, specialty, education, and hospital

bonuses/incentive, differential pay or other forms of compensation
