Registered Nurse License Certifications: BCLS Must-Have: - Ability
to communicate effectively in English, both verbally and in
writing. - Basic computer knowledge and word processing skills. -
Good interpersonal and communication skills. - Ability to use
verbal and nonverbal calming techniques to prevent crisis
situations. Description: The Registered Nurse conducts patient
assessments and data collection in an ongoing systematic manner,
focusing on the physical, psychosocial, and cultural diversity of
the patient to determine care needs. The Registered Nurse applies
knowledge of illnesses, injuries, and diseases specific to the
population served and uses this knowledge in the assessment
process. The Registered Nurse develops a goal-directed,
individualized/patient-specific plan of care, prioritizes, and
utilizes resources available to the patient to support and execute
the plan of care. The Registered Nurse collaborates with the
patient care team in planning interventions to meet patient and
family needs. The Registered Nurse implements the plan of care,
executing nursing interventions in accordance with established best
practices, standards, policies, and procedures. The Registered
Nurse works with the Nurse Navigator to ensure continuity of care
and patient compliance with the developed plan of care. The
Registered Nurse evaluates and documents patient responses to care
and the outcomes of care provided. The Nurse revises the nursing
plan of care accordingly. The Registered Nurse determines if a
patient’s primary care clinician needs to be contacted during
treatment based on the patient’s medical status, medication review,
or care managed by the primary care clinician that may affect the
care delivered. The Registered Nurse identifies patients needing
services not provided by the eligible provider and documents the
process for directing the patient to the appropriate care setting.
The Registered Nurse redirects patients to a more appropriate care
setting if necessary. The Registered Nurse reconciles and documents
all medications for all patients treated in the clinic. The
Registered Nurse provides information about new prescriptions from
the provider to patients, families, and caregivers. The Registered
Nurse assesses patient, family, and caregiver understanding of
medications prescribed by the provider. The Registered Nurse
assesses patient response to medications on follow-up visits. The
Registered Nurse documents over-the-counter medications, herbal
therapies, and supplements. The Registered Nurse reviews the
patient’s pain management plan, consulting with the provider
regarding the effectiveness of the plan or the continued need for
pain control as appropriate. The Registered Nurse assesses
potential barriers to adherence as documented in the patient
teaching clinical flow sheet. The Registered Nurse provides care
instructions that include patient goals, disease and
medication-specific information, and self-care abilities. The
Registered Nurse provides the patient, family, or caregiver with
written instructions, including recommendations for self-care. The
Registered Nurse or designee provides educational resources or
refers patients, families, and caregivers to resources for help in
patient self-management. The Registered Nurse notifies the Nurse
Navigator of patients transitioning to other care settings.
