ChristianaCare, a distinguished leader in health services in Delaware, seeks a dedicated Clinical Pharmacist, Population Health to join our dynamic team. This pivotal role involves direct engagement in the development and execution of medication therapy management within a population health framework, focusing on optimizing patient outcomes through innovative, integrated care. The ideal candidate will drive our mission of delivering empathetic, advanced health services by ensuring the highest standards of pharmaceutical care across our diverse patient populations. Conduct comprehensive medication reviews and assessments to optimize patient care. Develop and implement evidence-based, patient-centered medication plans. Collaborate with healthcare providers to coordinate drug therapy across multiple settings. Monitor patient outcomes and adjust medication plans as needed. Educate patients and healthcare teams on medication use and best practices. Participate in research and quality improvement initiatives within population health. Strong knowledge of pharmacotherapy Expertise in chronic disease management Proficiency in using electronic health records Ability to work collaboratively in interdisciplinary teams Strong communication and educational skills Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current Pharmacist licensure in the state of Delaware or eligibility for licensure. Certification in Medication Therapy Management or similar is preferred.
