Job Description Job Description Summary: The Health Educator
Supervisor is responsible for meeting the contractual requirements
and work plan goals. They will provide oversight for the
Comprehensive Adolescent Pregnancy prevention Program, and Personal
Responsibility Education Program, while supervising CAPP/PREP Staff
: Youth Health Educators, Parents/Community Outreach Liaison, Media
Communication Specialist/Consultant, Interns and Peer Mentors.
He/She will act as a liaison to the community and promote issues of
comprehensive sexual health education, adolescent pregnancy
prevention, family planning and youth development. The HES will
report to the Director of Youth Services. Essential Duties and
Responsibilities: Responsibilities: Work with Elmcor Youth & Adult
Activities partners including Article 28 Health providers, schools,
Community -based organizations, Faith -Based organizations,
Legislators through our Queens to ensure program youth and families
are receiving required services. Provide information sessions to
individual youth or groups. Sessions can include parents, schools
and service provider staff to promote comprehensive sexual health
education and assist parents with communicating with their youth.
Develop positive activities that are youth development focused and
will support healthy choices, a healthy lifestyle expose youth to
career exploration, cultural and educational activities. Plans and
executes program activities in accordance with goals of CAPP,
Technical assistance from Act for Youth, Guidelines for Healthy
Foods in Ensure completion of all program activities and
curriculum, in compliance with approved work plans. Manages the
CAPP team in all areas of supervision, performance management,
hiring, and training staff. Provide bi-weekly supervision to CAPP
staff and Youth Peer Mentors. Refer CAPP youth for Elmcor and/or
Community extracurricular activities, recreation, job training,
employment etc. Works with Data Analyst in data collected from
needs assessments to create community plans. Assist in crisis
intervention of client needs including domestic violence,
education, legal, mental health, and medical issues. Manage and
assist in meeting the goals of the Community Advisory Council
Meeting. Maintain contact with current funders, contract managers.
Complete required monthly reports, quarterly reports, and
continuation of funding applications. Working with Data Analyst,
responsible for quality assurance including chart reviews, program
evaluations and reporting both internally and externally. Review
budgets and assist in budget modifications and adjustments
narratives Revise and update policy and procedures manuals, forms
and systems as needed Attend workshops/conferences and represent
Elmcor in the service provider community at various
meetings/function Other duties as assigned Competencies: To perform

Qualifications: To perform this job successfully, an individual

functions. Education and Experience This position requires
Bachelor’s Degree at minimum, with at least 3-5 years experience
conducting workshops with adolescents, parents, and the community
at large. Masters in Health Education or Public health preferred.
Strong knowledge and experience with adolescent pregnancy
prevention, program implementation and evaluation are required. The
position requires a flexible schedule that includes some
evening/weekend work. Computer literate and bilingual. Other Skills
