Non-Exempt THE ORGANIZATION: Net Community Care is a division of
Northeast Treatment Services (NET), a non-profit organization
serving the Philadelphia region. As a designated Community Umbrella
Agency (CUA) for the Department of Human Services, Net Community
Care operates CUA1 and CUA7, providing comprehensive child welfare
services focused on safety, permanency, and family well-being.
Grounded in trauma-informed care and cultural humility, Net
Community Care works to stabilize families, support youth
development, and ensure children can grow in safe, nurturing, and
permanent homes. The organization is committed to strengthening
communities through collaboration, accountability, and continuous
learning. SUMMARY: The Aftercare Worker at Net Community Care,
supports families who have successfully exited formal
case-management services through DHS/CUA. This role focuses on
reintegration planning, resource connection, and ongoing
consultation to reduce the likelihood of re-entry into the child
welfare system. The Aftercare Worker collaborates with case
management teams, children, biological families, and external
partners to ensure families are equipped with the tools and support
needed for long-term stability. Qualifications WHAT SUCCESS LOOKS
LIKE IN THIS ROLE: Success in the Aftercare Worker role means being
a reliable and empathetic support system for families transitioning
out of formal child welfare services. On a daily basis, the
Aftercare Worker builds trusting relationships, maintains proactive
communication, and collaborates with internal and external partners
to implement effective aftercare plans. Empowering families through
teaching essential life skills and connecting them to community
resources is central to the role. Accurate documentation and
consistent follow-up are key, as is the ability to adapt to each
family’s unique needs. Ultimately, success is measured by the
family’s ability to remain safely and sustainably independent,
without re-entering the child welfare system. MINIMUM
QUALIFICATIONS: Bachelor’s degree in social work or related field
required. Candidates with a degree in an unrelated field require a
minimum of three years' experience working in child, youth, and
family serving systems. Must have strong interpersonal skills and
the ability to communicate well orally and in writing. Must have a
valid driver’s license. ESSENTIAL POSITION REQUIREMENTS: Must

regulations. Free from contagious disease/infection Pennsylvania
Child Abuse History Clearance Criminal Check Clearance Mental
Health Assessment Substance Abuse Assessment Able to speak English
clearly and hear voice of conversational tones. May be required to
use own vehicle when other means of transportation are
unavailable.
